July 27 to August 8, 2015

Quotation should be submitted in a sealed envelope until July 15, 2015 and opening of bids will be on July 16, 2015, 10AM at PSA-Bulacan.

Conditions:
1. Bidders must be PhilGEPS registered.
2. Bidders must have an updated 2015 BIR registration, DTI registration, Mayors Permit.
3. With free flowing coffee.
4. With provision of purified drinking water/ water stations with available drinking cups.
5. With provision of cornick/candies in the afternoon sessions.
6. Bidders must provide garbage bag.
7. Bidders are encouraged to use biodegradable materials/utensils.
8. Provision of food buffer of 10% of the total number of pax per day per training area.
9. Responsible and accountable for any incidence of food poisoning (not included are cases wherein foods served intended for lunch/snacks taken 4 hours thereafter).
10. Meals must be delivered 10 minutes before meal time (10am, 12nn, 3pm)
11. Responsible in food management:
- Food distribution (to ensure that all participants receive their lunch and snacks)
- Ensure the availbility of coffee
12. Unconsumed foods served daily during snacks and lunch should be properly
turned-over in clean containers to the PSA-designated personnel.
Other Information
Payment will be made in check, and will be delivered only after satisfying the following conditions:
1.) after the training period;
2.) upon the availability of 2015 POPCEN funds; and
3.) after completing and providing all the necessary documentary requirements.

d.) Mayor's Permit

A tax certificate will be in the form of BIR 2306 and BIR 2307 unless the supplier has tax exempt certification.
