6123345 Procurement of Security services from May to December 2019 for PSA RSSO V, Albay, Camarines Sur, and Catanduanes

Terms and Conditions:
1. Only the suppliers registered at the Philippine Government Electronic Procurement System
(PhilGEPS) shall be allowed to submit the quotation.
2. All entries must be typewritten/printed legibly in the Bid Form. Failure to use this form will
result to disqualification of your bid.
3. Late submission of quotation shall not be accepted.
4. Bids exceeding the ABC shall be disqualified.
5. All Interested bidders shall submit the following eligibility and technical requirements together with their
bids.
1. Registration Certificate from Securities and Exchange Commssion (SEC) or Depratment of Trade
and Industry (DTI);
2. Valid and Current Mayor’s/Business Permit;
3. Statement of the prospective bidder of all its ongoing and completed government and private
contracts within the last three years prior to the deadline for the submission and receipts of bids, including contracts awarded but not yeat started, if any;
4. Audited Financial Statement, showing among others, the prospective bidder's total and current
assets and liabilities, stamped received by BIR or its duly accredited and authorized institutions, for the preceding calendar year which should not be earlier than two (2)
years from the date of bid submission;
5. Computation of Net Financial Contracting Capacity (NFCC);
6. PhilGEPS Registration Certificate;
7. Omnibus Sworn Statement;
8. Tax Clearance;
9. Computation of Bid Price / Cost Distribution per Month;
10. PADPAO Certificate/Registration vs DOLE Rate;
11. Licenses of Firearms.
6. Bidders with insufficient or lacking eligibility and technical requirements shall be automatically disqualified.
7. Award of contract shall be made to the lowest quotation, and complies with the specifications
and other terms and conditions as stated in the RFQ.
8. Terms of Payment shall be made through check payable to the supplier.
9. The PSA reserves the right to reject any or all bid proposals, or declares the bidding a failure,
or not to award the contract, and makes no assurance that a contract shall be entered into as
a result of this invitation.
10. The Lowest Calculated Bid shall undergo Post-Qualification (Remittances to SSS et. al.)
11. To guarantee the faithful performance by the winning bidder of its obligation under the contract, it shall
post a performance security in accordance with the form and amount specified in Sec. 39 of Revised
IRR of RA 9184 prior to the signing of Contract.
12. Online submission of bid/s will not be accepted.
