Approved Budget for the Contract: Eight Hundred Thousand Pesos Only (P800,000.00).

Project Date of Completion: December 29, 2016

Eligibility Documents Required:
1. Mayor’s Permit issued by the city or municipality where
the principal place of business of the prospective bidder is
located.
2. Registration certificate from SEC, Department of Trade and Industry (DTI) for proprietorship, or CDA for cooperatives, or any proof of such registration as stated in the Bidding Documents. (SEC Registration certificate shall be supported with the Articles of Incorporation).
3. Updated PhilGEPS Registration Form
4. Tax Clearance per Executive Order 398, Series of 2005, as finally reviewed and approved by the BIR.
5. Statement of the prospective bidder of all its ongoing and completed government and private contracts. (Shall include all contracts for three (3) years within the period 2014 to 2016).

Terms and Conditions:

1. Only the suppliers registered at the Philippine Government Electronic Procurement System (PhilGEPS) shall be allowed to submit the quotation.
2. All entries must be typewritten/printed legibly in the Bid Form. Failure to use this form
will result to disqualification of your bid.
3. Late submission of quotation shall not be accepted.
4. Bids exceeding the ABC shall be disqualified.
5. The Lowest bidder shall be informed immediately and shall be asked to submit
additional requirements within three days after the opening of bids or during post qualification.
6. Award of contract shall be made to the lowest quotation, and complies with
the specifications and other terms and conditions as stated in the RFQ.
7. Terms of Payment shall be made through check payable to the supplier.
8. The PSA reserves the right to reject any or all bid proposals, or declares the
bidding a failure, or not to award the contract, and makes no assurance that a
contract shall be entered into as a result of this invitation.
9. Preparation and payment of documentary requites (permits, clearances, plans, etc.)
pertaining to the completion of project shall be included in the contract price.
